                              PRE-SESSION INSTRUCTIONS

In-Office Treatment Sessions   (Scroll down for In-Your-Home sessions)
 

Expect to be in the office for approximately 2 hours, to allow time for greeting, changing clothing, payment, re-scheduling, and discussion.

Before your appointment day, print the 5 attached forms (if you have a printer), read them carefully, and write down any questions that come to mind, either on each form or on a separate piece of paper.  These are not able to be filled out on your computer.
If you do not have a printer, please just read them all carefully and jot down questions.  You can fill out the forms when you arrive. 
Bring all 6 completed forms and your questions with you to your appointment to discuss with me before signing them.
Wait to sign and date all forms until after you are here for your appointment and we take time to discuss any questions you have.
Do fill out the New Patient Information & Medical History form if you were able to print it.  If you wish, you may scan and email it back to me to review before your session if you wish, but only with full awareness that this is not a secure method of transmitting this PHI (Protected Health Information), and that you are waiving that HIPAA right in this instance. 

Wear (or bring) elastic-waist loosely-fitting shorts, and a sports bra for women, or you can be treated wearing your underwear, as many people find easier, if that is comfortable for you. 
As described in more detail in my Informed Consent form (attached), during MFR evaluation and treatment, I will be observing and touching your body, and treatment is done directly on your skin. 

Please bring a mask if you choose to wear one, or one can be provided for you to wear.  I may wear one at times to protect you and my other patient-clients.

Please take measures to minimize any strong scents or odors on your body and clothing, including tobacco, perfume, or cologne, for the comfort and welfare of myself, others in the office, and my next patient-clients, many of whom may have allergies or sensitivities.

 

Do not arrive for your appointment under the influence of any recreational drug or alcohol or having recently used any tobacco product.
It would also be ideal, if possible,  to avoid arriving dehydrated or having just eaten a large meal.

In-Your-Home Treatment Sessions
 

Expect that I will be in your home for a minimum of 2 hours, to allow time for greeting, payment, setting up my treatment table, discussion, re-scheduling, and packing up the table.

Before your appointment day, print the 5 attached forms (if you have a printer), read them carefully, and write down any questions that come to mind, either on each form or on a separate piece of paper.  These are not able to be filled out on your computer. 
Have all 5 forms and your questions with you when I arrive, so that we can discuss any questions that you have about them before you sign and date them.
Do fill out the New Patient Information & Medical History form before your appointment time, if you are able to print it.

As explained on my website, please have a clear space available for me to set up my treatment table, preferably on the ground floor, in a quiet room where you will have privacy and we will be undisturbed by pets, other household members, ringing telephones, or doorbells that need to be answered. 
Please locate this near some type of table, desk, or counter, where I can place paperwork and take notes during the session.

I also request that you have 2 bed pillows and a sheet or blanket to cover your body if you become cold during the session, since I will only bring one sheet to cover the table for you to lie on top of.
