Informed Consent and Office Policies.

of this PATIENT FORMS page.
These will take some time to read,
but you will sign these when you arrive
for your appointment.

After scheduling, 
return to this PATIENT FORMS page,
read the Privacy Practices form
and the Infectious Diseases/COVID Policy form.
Then go to the Medical History (Intake) form.
If you have a printer,
print and complete only the Intake form 
to bring to your appointment.
These instructions and links will also
be emailed to you
after you book
your Initial Evaluation & Treatment session.
